Final Forms will ready for the 24-25 school year on June 7!! Login and update or register. Students using school email addresses will need to log in through the web, not school apps. Fall athletes need to have physical and medical release in by August 2nd.
You will not receive any final forms communications from coaches until you register for the sport.
Athletes should be registered with final forms and have a physical and medical consent on file PRIOR to sports beginning. Forms can be found in the pdf attachment. Physicals must be dated after 4/1/24.
